The ball extension would be the only one that would give you extra room. If you think about it, with a coupler extension on, and a trailer @ 90 deg. to the truck, you've done nothing to add clearance. However, with a ball extension @ 90 deg., you've moved the trailer back 4".

With a "ball extension" one might as well mount the ball in the truck bed the several inches rearward instead of paying for the extra. This does move the applied load.

On GM trucks this is not an option as the crossmember for the bed is above the axle. So the ball cannot be. I know this cause I just put one in less than a month ago. I bought the extend-a-ball, it's called from B/M.
