I am looking to get a rear loading ramp installed on a 2004 Sundowner 4 horse slant gooseneck. I am in northern Colorado, so was wondering if anyone might know a dealer or fabricator that can do this work for me.

I would check with Scott Murdock Trailer Sales or Transwest. I think both dealerships are north of Denver. They both have large shop facilities, and should be able to get a ramp installed for you.
