just so you know ,even with a sign form the people  can sue. If you insurce company gets involved they can say that you are not  cover for commeical use. I only help friend out that I know very well- this is my 2cents

I have one that was drawn up by a friend of mine who is an attorney and owns horses.  I'm not a professional hauler either, but I use it when I'm hauling my friend's horses.  It's 3 or 4 pages and covers just about any situtation you could think of.  I don't have it in an emailable format.  If you'd like me to fax it to you tomorrow, send me a PM.
