Assume you really mean full wheel covers as most have them.
Sometimes the tabs of the covers just don’t bite into the wheel firmly when initially installed. The normal flexing on the wheel will unseat them.
Just before every trip, take a rubber mallet and go completely around the cover and make sure they are completely flush against the wheel - all the way around. After several of these the tabs will flip over so they are pulling inward instead of pushing outward.
I have a box trailer with covers like that, and lost a couple before seeing the issue.
Now I carry a rubber tire mallet and reseat them all when doing the walk around on the tires.
Haven’t lost any lately.

After replacing many of mine on my bumper pull trailer I am considering going with a different style like my living qtrs trailer that uses a "lug nut cover with jam nut". It isn't cheap but in the long run it makes up for it as you aren't replacing them all the time. You can google "Phoenix Wheel Simulator", they are a good source for this and appear to have reasonable prices.
